Waiver

1. ASSUMPTION OF RISK

I understand that participation in martial arts involves inherent risks, including physical contact, striking, kicking, grappling, throws, falls, collisions, and use of training equipment. Injuries may include bruises, sprains, strains, fractures, concussions, or more serious injury.

I voluntarily choose to participate (or allow my child to participate) with full knowledge of these risks.

2. RELEASE & LIMITATION OF LIABILITY (NEW YORK COMPLIANT)

To the fullest extent permitted under New York law, I agree that Virtues Martial Arts LLC, its owners, instructors, employees, and agents shall not be liable for injuries or damages arising from the inherent risks of martial arts training.

Nothing in this agreement is intended to waive rights that cannot be waived under New York law, including claims involving gross negligence or willful misconduct.

3. MINORS

If the participant is under 18, I certify that I am the parent or legal guardian and consent to my child’s participation. I understand that New York law does not allow a parent to waive a minor’s personal injury claims, but this agreement reflects informed consent and assumption of risk.

4. EMERGENCY MEDICAL CONSENT

I authorize Virtues Martial Arts LLC to obtain emergency medical treatment for myself or my child if necessary and agree to be responsible for related costs.

5. MEMBERSHIP & PAYMENTS

Registration fees, tuition, equipment, camps, birthday parties, testing fees, and private lessons are non‑refundable
Membership cancellation requires 30 days’ written notice and payment of the final month’s tuition
Accounts must be current and in good standing to participate in classes

6. PHOTO & VIDEO RELEASE

I grant permission for Virtues Martial Arts LLC to use photographs and/or videos of myself or my child for promotional and educational purposes, without compensation.

7. LEGAL TERMS

If any part of this agreement is found unenforceable, the remainder shall remain in effect. This agreement is governed by the laws of the State of New York.

8. DIGITAL ACKNOWLEDGMENT & SIGNATURE
